Individual transition plan

For all pupils in form 8 who have been assessed as not ready for upper secondary education, an individual transition plan will be made.

The individual transition plan must ensure that a specific and tailored school and counselling effort is put into motion so you can grow academically, personally and socially and become ready to attend upper secondary education. The individual transition plan is made by your parents, your teachers and the educational guidance counsellor.

Goals related to your personal, social, practice-based and/or academic prerequisites are set up in your individual transition plan. These goals will help you maintain a focus on the skills you need in order to become ready for the upper secondary education you want to attend.
